Advice needed on first time server migration project

Hello TechNet...

At my place of work we have three Windows Server 2003 Standard Edition SP2 servers which we are trying to amalgamate into one Windows Server 2012 R2 AD server.  Each of these servers has specific roles installed:

- Server 1: File Server, Application Server, Terminal Server

- Server 2: File Server, Print Server, Primary Domain Controller (AD), DNS Server, DHCP Server, WINS Server

- Server 3: File Server, Application Server, Secondary Domain Controller (AD), DNS Server

My boss has asked me to write a "Server Conversion Plan" that documents all the necessary steps on how to amalgamate the above servers into the one 2012 R2 server.  The rather large issue I'm having is that I've never worked with servers before being hired here and so I haven't the faintest idea of where to even begin tackling this project.  The only thing I know at the moment is that I will eventually have to migrate our current Domain Controllers to our R2 server so that we have only one functioning Domain Controller.  That said, I have no idea what I'm doing when it comes to the other roles on each server... my guess is that, like the Domain Controllers, I have to probably migrate them as well?.. Aside from these issues, what do you do with the hard drives in these machines?.. and what about all the network shares, software, and databases that's installed on them?.... (I'm probably just scratching the surface aren't I?)
